Please type-in all relevant details as requested in the form and make sure you initial every page in the bottom left corner.

Not all applications are successful and we cannot process applications without an authorized signature, company stamp and a copy of your Trade License or Commercial Registration issued by the local Department of Economic Affairs or Local Municipality.

Please fax all nine pages of the signed form and a copy of the Trade License or Commercial Registration to our Accounting Department under +971 4 3063-111.
